Tell us about your nominee’s outstanding customer service accomplishments. Please include information relating to the customer service attributes listed. The information you provide will be used by the Review Committee to determine the recipient of this award. Please limit your description to 700 words or less.

  • Initiation: Attention is focused on gaining consensus for the need to change and then creating a vision and a plan for achieving it for the customer.
  • Helpful: Honor your commitments in a timely manner.
  • Implementation: The action plan is put into practice in order to achieve intended and measurable result.
  • Institutionalization: New improvement is integrated into policies, budgets, and ongoing programs.
